Unless the exisitng hole is a misshaped mess

yes, although you can sometimes use a slightly larger holesaw on its own first to make it round

why dont manufacturers make odd shaped downlight that needs a random size / shaped hole, specially for replacing them when someone has made the hole with a hammer?
