Sounds to me as though some one some where has done a test incorrectly, or replaced a connection incorrectly after doing a test, and the old assuming something is right without actually confirming 100% that it is right. As Murdoch has pointed out, so far there has been lots of talk of test this and test that, but there have been no actual test readings given? What are the continuity readings along each conductor, through the switch, through the element etc. I would adopt the approach of forget everything you may or may not have done and start back at the beginning and test everything from supply to load via all switches. Assume nothing, as that is where most people mess up fault finding by assuming something that is wrong.
