For contract cancellation under the "The Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013" please?

Struggling to find my version now, don't know what I've done with it & it's rare I need one!

Model cancellation form. Very basic outline I use. Don't know if it's any use to you?

Name of consumer(s),

Signature of consumer(s) (only if this form is notified on paper),
