Maybe she would have survived if she had had shoes on. I read it that it was an inline connector wired the wrong way round. My father in law did that once and somebody working in his house got a belt of it luckily he survived.

Went to a job at an old mill once. Mill had 3 phase supply but no metering. Owners son had wired directly into one of the phases to an old round pin plug top (15A) which he connected to an old socket to feed a load of outhouses (for free). Luckily you could see the cable route to the plug top and nobody had unplugged it!
