It looks like a TNC-S and a TNS, Its a very old head and I suspect that it has been intentionally "improved" from an original TNS to TNC-S at some stage.

 
Could there be a fuse in that N conductor?
Unlikely, I would have thought. If they had it apart to convert to TN-C-S, then they would probably have replaced any neutral fuse with a solid link.

 
Ahh, but you are spoilt on the IOW you have a good DNO!!!
And a high proportion of our meters have a quarter-turn isolator at the bottom right-hand corner, too. The seal fairies here are mostly out of work. :^O

 
thats a bit of a contradiction
it was i think andy just typed fast ROTFWL

unless he meant earthing the sheath the few meters or yards to where the sheath earth has broken down

it was worded badly...

the earth to the property is from the neutral, most likely because the sheath earth has failed somewhere. the sheath is most likely only being earthed by the connection to neutral at the service head

and its also a possibility that the next house is TNS, from this TNCS connection
