2659 & 2664, customer allowed us to clean out the panel and block the hole, but would not allow the replacement of the fuse holder and damaged wiring.

0529, this type of gland has ridges on the inside of the clamping faces, these can easily cut into the singles through vibration etc. customer would not allow repairs!

Typical short sighted approach, its not broken as long as its still going. No thoughts about preventive maintenance or improving equipment reliability. no doubt he'll be on the phone before too long begging you to come and fix it. make sure you charge him

 
If the customer had a fire caused by sparking (failed insulation or contactor arcing), and as you are the last competent person to have worked on the customer's equipment, who would carry the blame for the fire?

The yellow phase on the fuse carrier looks like it has had a bit of heat through it.

 
Customer has been advised in writing of the issues including these pictures.

They have also been sent to the customers H&S dept.

I have treaceability on the communications.

They have to date not responded to request repairs.
