danger notices for elecsa

do elecsa/eca do these forms or do you make them up yourself if so what needs to go on them? Obviously it needs to say

7671 does not contain a 'danger notice', so its not a form to 7671. so no requirement of any info that goes in there

i print them myself onto NCR paper

iirc, NIC do, but i dont think elecsa does
