Supply a TP&N commando socket in 4 core swa cable using the metal as the earth, how do you attach the bango and earth at the commando end?

 
Inside the SWA gland pack you will see a brass item with one large hole (same size as the Gland) and a small hole. This is called in the trade a banjo, I presume because it is shaped like one, but have no idea who first coined the phrase.

SWA cable can be terminated into plastic, and the old method of connecting the earth was to use the banjo with a coach bolt, or roofing bolt as a terminal.

Newer innovations have replaced the banjo with what @Andy mentioned above, a pirana nut. This goes onto the SWA gland thread and has various screw terminations to assist in connecting the earth fly lead to the earthing terminal.

You have to be very careful connecting SWA cable into any plastic accessory. If the cable is not "dressed" correctly at entry, it will distort the plastic and may effect the IP rating.

 
I don't like tightening up banjo's onto plastic boxes the box will always give a little and the hole for earth bolt defeat's the IP rating 

so try what I said, or the pirana nut,  no hole to drill so no problem with IP rating

It is better to trap the banjo between two locknuts or rings as trapping it between the gland body and the plastic box could loosen over time as the plastic can give way over time or becomes soft when the temperature rises. 

 
As has been suggested a plastic box is just that, it's plastic, not solid, so when in compression it will undergo plastic deformation, or creep under compression, this, will reduce the clamping forces on any face to face seatings.

 
Don't drill the top as water could get in , drill the bottom  or bend the banjo up for a pin & nut .

Put the banjo on the inside , some people do anyway , theres usually a way .

hole top or bottom  does not matter when its comes to IP rating's like the IPX5 and above 

as for two lock nuts plus the box wall and the banjo, I normally run out of thread

I have for some time now been putting the banjo on the inside of metal boxes as they can be seen better and keep cleaner

Do you use brass locknuts with them if they are on the inside? I was taught that the banjo should always be in brass - brass contact with the gland. 

 
No I don't use brass unless they come with the gland

I don't really see why we need to use banjo on metal boxes anyway if the gland is done right (tight and clean metal) if the gland is lose then the banjo wont work anyway

 
As others have already said, never tighten the gland directly onto plastic when using the swa as the cpc. Always sandwich the banjo between the gland and a locknut first. Many newer items such as Pirana nuts are also availible but these can not always  be used if the thread on the case is deep. Sometimes its easier to gland off into a steel box first.

yes you are right I was just saying you could use the MK thingy it has a thread ,"O" ring and an earth cable , so pretty water tight for the plastic box, it just looks better on the wall than a 4X4 glav box and socket below/above
